Please arrive with your ID and any insurance or other documentation you made need, and a list of current prescriptions or treatments

Depending on the appointment type, please be sure to follow any instructions provided by your provider

A list of forms is available here you will need a pdf reader to open these forms. These include the New Patient Packet, the Release of Health Information form, and the Designated Party Authorization. If you prefer to have these forms emailed, please call or text 360-748-0211. If you prefer to receive a hardcopy prior to your visit, stop by our office to pick them up, or request that they be mailed to you. Please allow for 3-5 business days for mailing of forms.
